Palliative Care

Palliative care is specialized medical care for people with serious illnesses, focused on providing patients with relief from the symptoms, pain, and stress. No matter the diagnosis, our goal is to improve quality of life for both the patient and the family. Our services are appropriate at any age and at any stage in a serious illness. Palliative care can be provided together with other medical treatment for the underlying illness.

Schneck’s palliative care team addresses the full spectrum of quality of life needs including pain and other symptom management needs, emotional, psychological, and spiritual problems, family support, advance care planning, and helping patients and families make difficult decisions about medical care in light of progressive illness.

Palliative Care Team

Our multi-disciplinary team works together to provide a comprehensive care plan. Members include:
  • Medical director, board certified in family medicine
  • Palliative care certified nurse practitioner

  • Palliative care certified home health nurse

  • Two Nurse Practitioners with pain management experience
  • Oncology certified nurse from the Cancer Center
  • Respiratory Therapist
  • Social worker

  • Chaplain

  • Psychologist

Care Coordination

Your palliative care team will work with your primary care physician to combine pain and symptom control in all aspects of your care plan. Palliative care supports both the patient and family and can improve coordination of care with other health care providers. The members of Schneck’s palliative care team provide open discussion about treatment choices, even when tough decisions need to be made.

Insurance Coverage

Insurance coverage is an important consideration for most patients. Depending upon the diagnosis and severity of the condition, most insurers offer benefits when medical necessity is established. Schneck participates in most major insurance plans. Every insurance plan is different, but we will be happy to work with you to pre-authorize the therapy if needed.
