Looking for a primary care doctor? New patients are encouraged to contact the office directly. Give us a call! We're happy to connect you with a Primary Care provider that suits your needs. Check our our new patient letter and complete the new patient form and bring it with you to your first appointment. You may also need an authorization form to share information about your care with other providers.

Schneck Pediatrics: Monday-Friday 8:30am-Noon and 1-4pm for established patients
Schneck Family Care - Jennings County: Mon-Thursday 8a-5p, Friday 7a-4p, Everyone welcome
Schneck Family Care - Scott County: Monday - Friday 8a-5p - Everyone welcome

Medicare Annual Wellness Visits
Once a year, Medicare offers patients an Annual Wellness Visit - an extended and in-depth appointment, including health screenings, to help identify problems that may not have been discussed during routine visits with the patient's primary care provider.
