Step 1 - Assess Your Needs
Consider you and your family's current health needs. If you need routine or preventative care, or a good starting point to determine next steps, be proactive and look into getting a primary care provider at Schneck.
Step 2 - Consider the Patient's Age
Consider the age of you or your loved one before choosing an area of focus in primary care.
- Pediatrics: typically treats patients between the ages of infancy and 21.
- Family Medicine: treats patients through all life stages from newborn to elderly.
- Internal Medicine: treats medically complicated adults and children.
Step 3 - Schedule an Appointment
Once you've made your decision, contact Schneck Medical Center, and we'll connect you with an available primary care provider. Our facilities are located in southeastern Indiana, making us an ideal healthcare provider for patients who live nearby.
New Patients
New patients are encouraged to contact the office directly. We're happy to connect you with a primary care provider that suits your needs. Check out our new patient letter and complete the new patient form. This form is essential, so please bring it with you to your first appointment. You may also need an authorization form to share information about your care with other providers.
Medicare Annual Wellness Visits
Once a year, Medicare offers patients an Annual Wellness Visit - an extended and in-depth appointment, including health screenings, to help identify problems that may not have been discussed during routine visits with the patient's primary care provider.
